Special Needs Programs Demystified
Certain Medicare Beneficiaries have illnesses that are Chronic in nature, have Income Restrictions or require institutional care. For these individuals Medicare has developed Special Needs Plans (SNP's)
Currently we offer Special Needs Plans from Aetna, Humana, Wellcare, Clover and UnitedHealthcare®.
Special Needs Plans have special features designed to accomodate the special needs groups of that particular group. Most noteably SNP's have all the features of Medicare Advantage (Part C). All SNP's are network based and require referrals from a Primary Care Physician to get services from specialists in the plan network.
Examples include Chronic Special Needs Plans (C-SNP) for individuals who live with Chronic conditions like Diabetes, Dual Eligible Special Needs Plans (D-SNP) for individuals dual eligible for Medicare and Medicaid and the Institutional Special Needs Plan (I-SNP) intended for beneficiaries living in nursing homes or assisted living.
If a beneficiary on Traditional Medicare or Medicare Advantage qualifies into a Special Needs Plan they are eligible for an immediate Special Enrollment Period and do not have to wait until the Annual Enrollment (Oct. 15th-Dec. 7th) or Open Enrollment (Jan. 1st-Mar. 31st)
